Data EZFacility needs from EZIDebit

In order to attach your EZIDebit account to your EZFacility account we require some specific information about your account. 

Please see the attached document and provide us with the requested information. It is recommended that you forward this email to a representative at EZIDebit and ask them to fill out this document for you. After the attached document has been completed, please send it to our support team by attaching it to a trouble ticket. Below are instructions on how to submit a trouble ticket. Setup may take a few days.
